Bangladesh's maternal mortality fell for a decade and then plateaued, and the stalled cause is the detectable one. Deaths from preeclampsia and eclampsia stopped falling and now make up about a quarter of maternal deaths, three-quarters of them rural. And the finding that defines the product: the verbal autopsies show that almost all the women who died had left home and sought care, most at more than one facility. Each facility met the dying woman as a stranger, with no history, no blood-pressure trend, no warning she was coming. These are routing and information deaths, the same disease the plastic bag has, at the moment it is most lethal.

The woman who today reaches her third facility as a stranger arrives instead as an expected patient at the right first one.

Nothing in Maa is a new kind of record. It is the same patient-owned, portable, signed record that turns a bag of paper into a person, applied to the one history whose absence is measured in lives. The surveillance is the spine doing what it always does, carrying a verifiable history to the next provider, except here the next provider is an emergency obstetric ward and the history is a blood-pressure trend that arrives in time. Maa detects and routes; it does not treat. But the stranger at the third door is exactly the failure a record that travels was built to end.
